All paperwork, collection devices and even the CryoChoice box are labeled with your unique, bar-coded number. This provides positive identification between your computer record, paper forms and your actual specimen.

You must ship out your sample so it arrives at our lab Tuesday through Friday.

How To Collect & Ship

The specimen collection cup is sterile and only opened just prior to use. Once it is filled with your semen, you will immediately pour the preservative into it. The preservative will keep your sperm stable during transportation. You do not need to refrigerate the sample after it is combined with the preservative.

After completing your donation, place it into the CryoChoice box. Slip the box into the plain FedEx envelope provided. A pre-paid and pre-addressed FedEx shipping label is included at no extra charge. Take your CryoChoice box to your nearest indoor FedEx/Kinkos drop off location. Once at the lab, your sperm is extracted from this media. It is then tested, processed and permanently stored.
